From the 1st October 2021, The Botulinum Toxin and Cosmetic Fillers (children) Act 2021 was introduced, making it a criminal offence to administer Botulinum Toxin (hereafter referred to as Botox) or a Filler by way of injection for a cosmetic purpose, to a person under the age of 18. It is also an offence to make arrangements to book an appointment to provide these treatments to anymore under the age of 18.

Dermal Fillers

Dermal fillers are injectable treatments which aim to soften the appearance of fine lines and wrinkles, as well as restoring definition and volume loss of the face.

Injectable Hyaluronic Acid (HA) is injected into the skin, HA attracts water, which helps to restore volume, recreate lost structure, and replenish our bodies’ natural sources of HA. This reduces the overall sunken or sagging appearance of the face revitalises and adds volume to the skin which softens the overall look of fine lines and wrinkles.

Dermal fillers are non-permanent because the body usually breaks them down over a period of time (9 to 18 months).

Types of Dermal Filler

Dermal fillers differ in chemical makeup, and longevity and have varying degrees of softness . Softer fillers are used in the lips, for example, while denser fillers might be desired to enhance cheekbones.

Lip Augmentation

Lip augmentation can be achieved when dermal fillers are injected into the lips to create volume and shape.

Dermal fillers for the lips have a softer G prime which helps to mimic the natural anatomical structure of the lip. Using either a needle or cannula, HA is injected to create shape, contour and restore volume. The amount of dermal filler is tailored to the client. Lip fillers can be used to reduce signs of ageing and smokers’ lines.

Dermal fillers for Cheeks and Mid-Face

Dermal fillers can be injected into the cheeks to create a sense of volume (fullness) and improve the definition of the cheekbones and facial symmetry. This can have a secondary effect of lifting the eye area and reducing the depth of the nasolabial folds as the face is gently ‘lifted’. Fillers in the mid-face can add definition and restore fullness to the face, lifting your cheeks to reduce the appearance of sagging.

Dermal fillers for Chin, Jaw and Lower-Face

Dermal fillers can be used in the chin, jaw and lower face to help to restore fullness in areas that have lost volume with age or to create a more symmetrical or balanced profile. As we age, our chin can lose volume and recede into the jawline, which can compromise the balance and profile of the face.

Dermal fillers can be injected into the jawline and chin to create a more balanced profile. They can be administered to re-sculpt this area to rebalance your profile, giving you a more youthful, symmetrical look.

Dermal filler can also be used in the lower face to address signs of ageing, for example, to restore lost volume or to reduce the appearance of lines and wrinkles (like our nasolabial folds and marionette lines) to create a defined jawline.

How Dermal Filler Treatment Works

Dermal filler works in two ways. Firstly, the gel is injected into the area to create volume or definition. Your practitioner can adjust the volume by adding more filler and sculpting the filler to get the look you desire.

The Hyaluronic Acid (HA) in the filler also boosts volume by attracting water, which helps to replenish the moisture in your skin. This leaves the skin looking plump and fuller, with a hydrated glow.

Preparing for Filler Treatment

You may not be suitable for dermal fillers if you:

Have a skin infection such as a rash, blemish or cold sore

Are currently unwell

Are pregnant or breastfeeding

Have previous nodules which may need dissolving

Have dermatological conditions such as rosacea

Recent infections or dental procedures within the last 2 weeks

Are travelling abroad within 2 weeks.

It is a good idea to schedule your appointment two weeks prior to any big event or travel. This will also allow any bruising around the area to fade.

You are advised to avoid any UV exposure for 2 weeks after treatment. Prolonged UV exposure can cause your fillers to break down so that they don’t last as long. UV exposure can also slow down the healing process.

What to expect from your Dermal Filler treatment

Before you have dermal fillers, you will have a consultation with your practitioner. This will allow you to discuss what you want and the realistic results you can expect to achieve.

Before treatment, a topical anaesthetic (numbing cream) may be applied; this is usually only in the case of lip fillers.

Do not take any anti-inflammatories as pain relief prior to your consultation. This could increase the risk of bruising.

Your practitioner will assess the area before administering the filler via a needle into the skin or cannula. You can expect some discomfort, particularly in sensitive areas like the lips, or in areas close to the bone, like the chin.

This discomfort should be mild and will resolve quickly. The procedure will usually take around 30-60 minutes to complete and your practitioner will monitor you after the treatment in case of dizziness or an allergic reaction (which is rare).

Dermal Filler Aftercare

To avoid the small risk of infection, the skin should be kept clean and clear after a dermal filler treatment.

Do not wear makeup for 24 hours post-procedure.

Do not rub or massage the skin. Try not to put any pressure on your face; for example, if you go to bed, try not to lie on your face for at least 48 hours after the treatment.

Avoiding touching or rubbing the area to minimise the risk of infection

Do not exercise or drink alcohol for 24 hours

Take paracetamol if in pain (and is safe to do so) and avoid ibuprofen as this may increase the risk of bleeding

Watch out for signs of infection and occlusion

Avoid UV exposure and apply SPF 50 to the face

It is usual to have some redness, tenderness and swelling around the injection sites, but this should dissipate after a few hours and disappear in 24 hours.

Bruising around the injection site is more common. You can apply ice and may be advised to take arnica. You can expect bruising to fade within 2 weeks

Dermal Filler Risks & Side Effects


The most common risk of dermal fillers is bleeding and bruising around the injection site. Bleeding should stop after the procedure. You can apply ice and may be advised to take arnica. You can expect bruising to fade within 2 weeks.

There is a small risk of infection; this should be limited by a skilled professional who will be trained in administering the treatment to minimise the risk. Keeping the area clean and free of makeup after treatment will help to reduce this risk further.

There may be some asymmetry in the face after the procedure, which can usually be corrected at 2 weeks.

Lumps under the skin or overcorrection of wrinkles (puffiness) may also occur

Rare Risks

In very rare cases, filler can cause vision problems and blindness.
Severe pain, blanching and discolouration to the area are signs of a possible Vascular Occlusion (VO). If caught early, the VO can be reversed by your practitioner. A VO is a medical emergency and you should reach out to your practitioner.

Removal of Dermal Fillers

Dermal fillers can be dissolved. Your practitioner may not be comfortable in removing filler that has been done at another clinic in cases where they are unable to verify the original product used. Some fillers may not be responsive to Hyalase and this can cause further complications.

Fillers should only be administered by a qualified and trusted practitioner.

  • Fillers are not permanent; they are made from substances which are typically broken down by the body over time. The length of time fillers will last will depend on the area and the type of filler used. Typically 3-12 months.

  • You will be asked what your desired outcome is and your practitioner will discuss the potential results based on a number of options. You will be advised on the type of filler and the amount of filler required. You will be taken through a medical consent form to ensure you are safe to be given fillers. Consent forms and photographs will be taken for medico-legal purposes.

  • In some cases, fillers can be dissolved. The effects of fillers is not permanent and will usually fade in 3-12 months. If you are nervous about what the results of dermal fillers will look like, your practitioner can use a small amount in your first consultation, and can build the results over time.

  • Dermal fillers can be used in the mid and lower face to lift the face. This can result in a lifting of the jawline and a reduction in the appearance of jowls.

  • Dermal fillers are naturally broken down by the body over time. Top-up treatments are required to keep the effects of fillers; otherwise, the face will return to it’s original state.

  • Dermal fillers work in a different way to wrinkle relaxation treatments. Botulinum Toxin A is used to relax carefully targeted facial muscles, temporarily reducing the appearance of fine lines and wrinkles. Dermal fillers can be used to restore volume, create definition or symmetry to the face. Your clinician can advise which treatment is suited to your desired outcomes. Botulinum Toxin A can be used in conjunction with dermal fillers to achieve a more youthful appearance.
